Massage Technician License

REQUIREMENTS AND INSTRUCTIONS FOR OBTAINING A MASSAGE TECHNICIAN OR MASSAGE ESTABLISHMENT LICENSE NOTE: MASSAGE ESTABLISHMENTS MUST FIRST OBTAIN A CONDITIONAL USE PERMIT FROM ZONING DEPARTMENT. Application Personal Affidavit $50.00 application fee Section 5.64.030 of the Glendale Municipal Code now requires a complete set of fingerprints. A fee of $52.00 is required, cash, credit card, or a check made payable to the "City of Glendale." NOTE: APPLICATIONS ARE ACCEPTED/PROCESSED ON TUESDAYS, WEDNESDAYS AND THURSDAYS - 9:30 A.M. TO 11:30 A.M. OR 1:30 P.M. TO 3:15 P.M. WHEN FINGERPRINTING IS REQUIRED. Five letters of reference from at least five (5) bona fide residents of the City. Two recent photographs, passport size (2" X 2"). Proof of 150 hours of training through a diploma, certified copy of a certificate of graduation or transcript sent directly to the City Clerk's office from a recognized School of Massage. A Doctor's certificate, issued within 30 days of application, stating that applicant is free of any contagious or communicable disease, including tuberculosis. ALL ITEMS LISTED MUST BE ACCOMPANIED BY APPLICANT Upon approval of application, a $100.00 license fee is required. License is from July 1 to July 1 and is pro-rated on the first one. To Renew a Message Technician Permit: Application Personal Affidavit $50.00 application fee Two recent photographs, passport size (2" X 2") A Doctor's certificate, issued within 30 days of application, stating that application is free of any contagious or communicable disease, including tuberculosis. Fingerprints are required every 3 years for renewals. Note above days and times when processed.
